Abstract/Description
Preventable readmissions are costly to hospitals. There is a need to hone in on proven methods to prevent readmissions; especially readmissions within thirty days. Research was performed to investigate nursing discharge planning and how it affects readmission rates. The goal is to provide evidence based research that shows a discharge planning program is beneficial to reducing hospital readmissions at Murray Calloway County Hospital. The incorporation of a new policy solely dedicated to discharge planning is to be recommended.
