Management of Hypertension in Adult Patients in the Primary Care Setting Quality Improvement Project
Project Abstract
The purpose of this project was to improve management of hypertension in adult patients in the primary care setting. This QI looked to develop an educational program concerning lifestyle modifications to implement along with pharmacologic methods for control of blood pressure and provide SMS-based support to patients. The goal was to increase patient knowledge of hypertension, as well as improve control. Improving blood pressure would help to decrease long term risks of cardiovascular disease, stroke, and MI. The goal of presenting this project will be to disseminate findings to advanced practice providers for application in the clinical setting. The poster will include background information, project objectives, methods and materials, results, data analysis, and discussion of the QI project.
