Abstract/Description

In this presentation, the purpose/problem, theoretical framework, evidence, and findings/conclusion will present information on comprehensive discharge planning that promotes safety and decreases readmission rates. The purpose/problem will identify today’s issues that comprise the high readmission rates that are present in health care facilities across the country. The theoretical framework will identify the methodology that was used to guide the research for evidence-based practice and studies that pertain to the presented problem. The evidence portion consists of studies that were found that relate to the problem that present solutions to the problem that can be implemented into practice that are backed up by evidence in other health care facilities. The findings/conclusion will establish the results of the research and how it will be implemented into the current policy and procedure for discharge planning.
