Abstract/Description

This evidence based practice project evaluated the effectiveness of implementing a policy of post-discharge phone calls within 24 hours of the first phone call and 72 of the second phone call by a nurse. The nurse provides assessment and education for the patient as well as answers any questions by the patient. Implementing the policy improves patient compliance, enhances satisfaction, and reduces unnecessary emergency department return visits. Evidence from multiple studies demonstrated that post-discharge phone calls significantly decrease return rates and improve patient compliance and satisfaction. Implementing this policy can have the potential to improve this facility's patient outcomes, improve satisfaction, decrease healthcare costs, decrease patient return rates, and improve the overall quality of patient care in the ED setting.
