The Meds-to-Beds program improves medication adherence by delivering prescribed medications directly to patients before hospital discharge, ensuring timely access and reducing barriers such as transportation or pharmacy delays. By incorporating bedside education from pharmacists, patients gain a clearer understanding of their medications, which enhances confidence and proper use. This coordinated approach has been shown to decrease readmission rates and improve overall patient outcomes.
